Request to Add Loans to a Federal Consolidation Loan
(180 day add on provision)

UHEAA also offers you the opportunity to combine additional eligible loans to your consolidation loan-up to 180 days after you consolidate with us. Please fill out the attached form and mail it back to us at UHEAA, attn: Account Services, P.O. Box 145112, Salt Lake City, UT 84114-5112. Authorization to Release Information to a Third Party Form

UHEAA takes your privacy very seriously and so should you. However, it may become necessary to have someone else contact us on your behalf. If you would like to request UHEAA to release information about your student account to someone other than you, complete the Authorization to Release Information to a Third Party Form and return it to UHEAA.

Automatic Payment Request Form

If you would like the convenience of having your monthly payment deducted from your checking or savings account complete our Automatic Payment Request Form. Mail the form to UHEAA at the address contained on the form. Once your request is processed you will receive a letter telling you when the automatic payment deductions will begin. Please continue to pay your monthly payments yourself until you receive this letter. You can also make a payment from your checking or savings account online through our website. Click here for Account Access if you want to make a payment online.

Remember when you enroll in the automatic payment program you will become eligible for the 0.25% reduction in your interest rate on Federal Stafford and Federal PLUS Loans. NOTE: You must be in repayment on your student loan(s) to qualify.

To sign up for automatic payments you will need to print the following form, complete it, and return it to UHEAA with proper attachments.

Child Care Provider Loan Forgiveness Program

The Child Care Provider Loan Forgiveness Program was enacted to encourage people to enter and continue in the child care profession. There are several qualifications that an applicant must meet to qualify, please read the instructions included with the application carefully before submitting the form to the U.S. Department of Education.

The forbearance form should only be used by individuals who previously received loan forgiveness under this program and are requesting additional loan forgiveness.


Application For PLUS Loan Credit Approval

If you would like to request a credit pre-approval for a Federal PLUS Loan, download, complete and fax this form to Utah Higher Education Assistance Authority at 801.321.7198. Your dependent’s school will determine your loan eligibility, if your credit is approved, for an academic period of up to 12 months.

The Teacher Loan Forgiveness Program was enacted to encourage people to enter and continue in the teaching profession. Benefits may be available to you if you are in the teaching profession.

Forbearance Form

Forbearance means permitting the temporary cessation of payments, allowing an extension of time for making payments, or temporarily accepting smaller payments than were previously scheduled. To request a temporary cessation of your payments, please complete this form and provide the reason for your financial hardship. Mail the form to your lender or the company that services your loan. You will be notified of the approval or denial of your request. Forbearance is granted at the lender's discretion.

Income Sensitive Request Form

An income sensitive repayment plan is an alternative to a standard level repayment schedule. An income sensitive repayment may reduce your monthly payment amount, but be aware that selecting an income-sensitive repayment schedule may also increase the total amount of interest you will pay on your loan(s). Income sensitive repayment schedules are approved in 12 month increments. To apply for an income sensitive repayment schedule, complete the application and submit the form with the required documentation.

Internship/Residency Forbearance Request Form

The Internship/Residency Forbearance is available if your two year Internship Deferment eligibility has expired or you are not eligible for an Internship Deferment because of the terms of your Promissory Note. This Forbearance is granted in yearly increments and you must re-apply annually. To qualify for the Forbearance you must complete and return the form to UHEAA.

Deferment and Discharge Forms

Deferment requests must be made by speaking or writing directly to your lender. To apply for a deferment, download the appropriate form here, complete and sign the form, and mail the form to your lender or the company that services your loan. Check with your lender or the company that services your loan for more information.

Some scholarships (and some loans) expect the recipient to work in certain fields (such as teaching) or provide certain services for a certain period of time or require them to pay back the funds received with interest.
